Intake call or visit
We sit down, in person or on the phone, and hear the whole story. Diagnoses, providers, current pain points.

Map the providers and the benefits
We organize who is currently involved, what each is responsible for, and what is being missed. We also review the benefit programs in play.

Build the care plan
A written plan. What is happening, what needs to happen, who is doing it, by when. This becomes the working document for the family.

Stay engaged
Regular check-ins, usually weekly or monthly depending on the complexity. The plan changes as things change.
Common questions
Do I need case management if my parent already has a doctor?
A primary-care doctor is necessary. Case management is different. It is the person who keeps track of the whole picture across specialists, pharmacists, therapists, home care, and benefits. For simple situations, one doctor is enough. For complex situations, case management saves the family hours every week.
Is case management covered by Medicaid?
Some forms of case management are covered under Idaho Medicaid for qualifying individuals. Private case management is paid directly. We can walk through what applies to your situation during the intake.
Can you work with providers who are not part of Abundance Living?
Yes. Most of the time we are working alongside external providers: primary care, specialists, therapists, pharmacies, other agencies. Our job is to connect the pieces, not to replace them.
